Player Waiver

In consideration of being allowed to participate in any way in the Shamrock Elite Hockey Tournament at Cadillac Ice Arena (Walpole, MA), Foxboro Sports Center (Foxboro, MA), Thayer Sports Center (Braintree, MA), Hingham Skating Arena (Hingham, MA), Boch Ice Arena (Dedham, MA), Bavis Arena (Rockland, MA), Lovell Ice Arena (Rockland, MA) , Bridgewater Ice Arena (Bridgewater, MA) or any other related events and activities of Shamrock Elite Hockey Tournaments, I, the undersigned, acknowledge, appreciate, and agree that:

1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown of my participation in Shamrock Elite Hockey Tournament at Cadillac Ice Arena, Foxboro Sports Center, Thayer Sports Center, Hingham Skating Arena, Boch Ice Arena, Bridgewater Ice Arena, Lovell Ice Arena, Bavis Ice Arena, SRJ Management, LLC, Express Management, LLC, Shamrock Elite Hockey, LLC, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation in Shamrock Elite Hockey Tournament at Cadillac Ice Arena, Foxboro Sports Center, Thayer Sports Center, Hingham Skating Arena, Boch Ice Arena, Lovell Ice Arena, Bridgewater Ice Arena, SRJ Management, LLC, ExpressManagement, LLC, Shamrock Elite Hockey, LLC. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS THE SHAMROCK ELITE HOCKEY TOURNAMENTS, Cadillac Ice Arena, Foxboro Sports Center, Thayer Sports Center, Hingham Skating Arena, Boch Ice Arena, Bridgewater Ice Arena, Lovell Ice Arena, Bavis Ice Arena, SRJ Management, LLC, Express Management, LLC, Shamrock Elite Hockey, LLC, their officers, officials, agents and employees, other participants, sponsoring agencies, sponsors, advertisers, and, owners, operators and lessors of premises used to conduct the event (“Releasees”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

5. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself, including, but not limited to, personal injury, disability, death, illness, damage, loss, claim, liability or expense, of any kind, that I may experience or incur in connection with my attendance or participation during or after the Shamrock Elite Hockey Tournament at Cadillac Ice Arena, Foxboro Sports Center, Thayer Sports Center, Hingham Skating Arena, Boch Ice Arena, Lovell Arena, Bavis Ice Arena, SRJ Management, LLC, Express Management, LLC, Shamrock Elite Hockey, LLC (“Claims”).

6. On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless Shamrock Elite Hockey Tournament, Cadillac Ice Arena, Foxboro Sports Center, Thayer Sports Center, Hingham Skating Arena, and Boch Ice Arena, Bridgewater Ice Arena, Lovell Arena, SRJ Management, LLC, Express management, LLC, Shamrock Elite Hockey, LLC, their employees, officers, managers, directors, agents, representatives, and owners, operators and lessors of premises used to conduct the event from the Claims, including all liabilities, claims, actions, damages, costs, or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Shamrock Elite Hockey Tournament, Cadillac Arena, Foxboro Sports Center, Thayer Sports Center, Hingham Skating Arena, and/or Boch Arena, Lovell Arena, Bavis Arena, Bridgewater Ice Arena, SRJ Management, LLC, Express Management, LLC, Shamrock Elite Hockey, LLC, their employees, agents, and representatives, whether a injury occurs before, during or after participation in Shamrock Elite Hockey Tournament, Cadillac Ice Arena, Foxboro Sports Center, Thayer Sports Center, Hingham Skating Arena, Lovell Arena, Bridgewater Ice Arena, and/or Boch Arena activities.
